400 and 403 errors in an embedded Google Form#525

I followed the instructions in the tutorial on embedding a Google Form in the wiki, but I get a 400 error where the embedded form should be. I also get a 403 error instead of the embedded form when accessing the same wiki page from an account other than the one I used to build the form. The wiki page is stored in a Shared Drive.
